2017 Legacy of Lowell Registration Form

The 14th Annual Legacy of Lowell Day of Service is an opportunity for University of Utah students, faculty, staff, alumni, friends and the greater community to engage with various Salt Lake City community partner organizations.

Lowell Bennion was a prominent leader in the Salt Lake City community. Lowell Bennion was responsible for founding the first food bank and homeless shelters in Utah. Lowell Bennion was known for gathering groups of students on Saturday mornings to address community needs such as visiting with and bringing food baskets to widows in the community and is also the namesake of the Bennion Center. On this day we honor his legacy.

This day of service will be held on Saturday, September 30, 2017 from 8:00 a.m. to 12:00 p.m unless otherwise stated on the project description. Legacy of Lowell is the official kickoff for Homecoming 2017!

There is limited parking on-site for those who want to drive to Glendale Middle School. Limited transportation from the University of Utah via school bus will be provided leaving from the Peterson Heritage Center, LDS Institute of Religion, and Rice-Eccles Stadium promptly at 7:45 AM to transport volunteers to Glendale Middle School.

Check-in, and breakfast will be held at Glendale Middle School (1430 W. Andrew Ave (1500 S.) starting at 8 AM.

All projects are on-site, unless specified with another location.  Please note that this year we have included whether or not the project is child friendly and if there is transportation to off-site projects. The Bennion Center and Legacy of Lowell committee highly encourage the use of green transportation such as public transportation, biking, or carpooling. Projects at the Glendale Campus will start promptly at 9 AM with off-site projects starting between 9:15 and 9:30.

Please look over the projects below and select one in which to participate Space is limited for some of the projects. Project registration is per person, so we ask parents to register their children separately.

Please remember to wear clothing appropriate for your project and we highly encourage you to bring a reusable water bottle and sunscreen or a hat.

Release of Liability

Forseeable and Unforseeable Risks

I understand that there are certain foreseeable and unforeseeable risks associated with any program or course activities conducted on or off campus. I understand that participation in Bennion center Activities, herein after “Activities” will require me/the minor Participant to travel to and from the above referenced sites and that I/the minor Participant will be responsible for arranging my/his/her own travel to these sites. I recognize that there are certain risks associated with such travel. I understand that circumstances including weather conditions, terrain, personal medical conditions, etc. may present risks over which the University has little or no control.

There may be other foreseeable and unforeseeable risks associated with activities. I am choosing for myself or for my minor Participant to participate in this program/course knowing of the foreseeable and unforeseeable risks. If I/the minor Participant become(s) concerned about any safety or security concerns during Activities, I/the minor Participant will bring these concerns immediately to the attention of Bennion Center advisors. I/the minor Participant can choose not to participate in Activities rather than to undertake the risks that cause me/the minor Participant concern.

Assumption of Risks

I will participate or authorize the Participant to participate Activities. I understand that such participation can include foreseeable risks identified above and unforeseeable risks and other hazardous activities inherent in the Activities which may expose the participant to illness, injury, or death. Participant or guardian/parent freely and voluntarily participates or allows participation in the program with the knowledge of the danger involved and hereby agrees to assume and accept any and all risk of injury or death.

Waiver, Release and Indemnification

  • Participant or Guardian/Parent of Participant understands and acknowledge that the University of Utah (“University”) is not an insurer of Participant’s behavior, actions or participation in the Activities, and that the University assumes no liability whatsoever for personal injuries or property damages to participant or to third persons arising out of Participation in the Activities. Participant or Guardian/Parent hereby agrees to release, waive, covenant no to sue, indemnify and hold harmless the University, and all of their officers, employees and agents (collectively the “Releasees”) from any and all liability, claims, demands, actions and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by Participant or loss or damage to any property belonging to participant arising out of or related to participation in the Activities, and excepting only such loss, damage or injury as may be caused by the sole negligence of any Release.
  • Participant or Guardian/Parent of participant agree that the site of any lawsuit arising out of or related to participation in the Activities shall be Utah and that this Agreement will be governed by and construed in accordance with the laws of the state of Utah, without application of any principles of choice of law.
  • Participant does not have any medical conditions that would prevent participation in Activities.
  • Participant has been advised to maintain her/his/their own health and accident insurance to cover the costs of treatment in the event of any injury.
  • Participant understands that he/she/they may be photographed and agrees to allow the photo, video or film likeness to be used by the Bennion Center for any legitimate purpose.
  • Participant shall pay any attorney fees or costs incurred by the University in enforcing this Agreement.

If any portion of the Agreement is held to be invalid by a court of law, then it is agreed and intended that all the remainder shall, notwithstanding, continue in full force and effect.
